REGISTERED NURSES

  • Provides initial and ongoing assessment and interpretation of patients or clients within the framework of holistic professional nursing practice, and in accordance with the principles of patient and family centered care.

  • Assesses and interprets information and data regarding the patient or client’s culture and values system, lifestyle, and interest and ability to participate in their care, and other factors impacting successful treatment outcomes.

  • Assesses and interprets patient or client’s ability (physical, social and psychological) to manage their health in order to transition their care or discharge. In collaboration with the patient or family identifies patient or family needs based on analysis of data.

  • Identifies and communicates patient or family needs to the interdisciplinary health care team and other providers of care as appropriate.

  • Provides ongoing monitoring of patient or client’s or family response to intervention; identifies deviations from expected outcomes and assesses change in patient status.

Qualifications:
   •    Current Certificate of Competency (licensure) from the College of Nurses of Ontario, is in good standing with CNO
   •    Nursing diploma or degree from a recognized college or university
   •    Valid CPR or BCLS Certificate
   •    Membership with RNAO/RPNAO an asset
   •    Knowledge of the community care and home visiting is an asset
   •    Knowledge of CVAD, Infusion Pumps, IV therapy, Wound Care is an asset
   •    Effective use of technology to document, monitor and communicate with clients, families and partners.
   •    Ability to integrate and apply critical thinking skills to client care
   •    Ability to recognize safety issues, follow safe work practices, advocate for client care and safety concerns
   •    Ability to practice nursing in accordance with client/family-centered care principles in establishing authentic/empathetic professional relationships with clients and families, understanding and documenting client/family preferences and goals, advocating for clients/families
   •    Effective collaboration and communication skills with other members of the health care team to ensure patient access services and smooth hospital-to-home transitions
   •    Demonstrated ability to use knowledge-transfer techniques to support practice delegation
   •    Demonstrated commitment to modelling lifelong learning and professional growth
   •    Ability to speak good English 
   •    Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, pushing and pulling

REGISTERED PRACTICAL NURSES

    •    Triage incoming patient calls. Assess patient’s health needs, discuss test results and respond to questions pertaining to health promotion and disease prevention
   •    Initiate the Electronic Medical Record (EMR) of the patient visit, purpose, history and health care needs. Record patient vitals and prepare patient and room for examination
   •    Educate patients regarding different tests, if needed.
   •    Maintain the EMR to recall patients for health promotion and disease prevention follow-up such as immunizations, pap smears, mammograms, FOB testing, bone density and diabetes care
   •    Administer and record immunization. Educate patients of possible side effects.
   •    Perform blood pressure follow-ups that include assessing the patient’s response to medications and providing patient education and self management strategies as per FHT program guidelines
   •    Maintain the INR monitoring and management system
   •    Complete pre-operative and MTO physical forms
   •    Maintain accurate, concise and confidential EMR documentation pertaining to patient history and encounter data
   •    Perform suture/staple removal, ear syringe and assist physician with minor clinic procedures.

 

QUALIFICATIONS:

    •     Current certificate of Registration from the College of Nurses of Ontario (RPN)
   •     Excellent patient assessment and triaging skills
   •     Knowledge of Immunization guidelines in Ontario
   •     Knowledge of and working experience with EMR systems and applications
   •     Excellent verbal and written communication skills
   •     Ability to work collaboratively as a member of multi-disciplinary team
   •     At least 1 yearPrimary Care Experience

PERSONAL SUPPORT WORKERS

  • Following the care plan, observing, and reporting any substantial findings and/or changes in resident behaviour to the appropriate registered member of the healthcare team

  • Working under the supervision of a Registered Healthcare Professional such as a Registered Nurse (RN) or a Registered Practical Nurse (RPN)

  • Demonstrated commitment in working with seniors

  • Experience in dealing with the cognitively impaired

  • Assisting with ambulation, positioning, and transferring using mechanical lifts such as a Hoyer lift

  • Assisting or providing total personal care such as toileting, bathing, and perineal care

  • Handle all other activities of daily living (ADL)

  • Assisting nursing staff by answering call bells

  • Performing observational care for confused residents
     

Qualifications:

  • Personal Support Worker (PSW) certificate from an Ontario Community College Program OR a private career college in Ontario that is registered with and approved by the Ministry of Training, Colleges and Universities with successful completion of the final examination administered by the National Association of Career Colleges (NACC) OR an approved Ontario Board of Education PSW Program

  •  Must have clinical and/or working experience as a PSW within the past two (2) years, preferably LTC

  • Ability to meet the physical requirements of the position including: Push/pull up-to 250 pounds, Lifting 50 pounds max, Working at/above shoulder height, Standing & walking for long periods of time, Frequent bending, twisting, reaching, crouching

  •  Excellent verbal and written communication skills

  • Ability to follow a care plan with minimal supervision

  •  Proof of current immunization for measles, mumps, rubella, and varicella (MMRV), tetanus, diphtheria, pertussis, and hepatitis b

  • Proof of negative two-step TB skin test within the past one (1) year or negative chest x-ray within the past five (5) years

  • Current BCLS-HCP (HealthCare Provider Level)

  • Current N95 Mask Fit Test Certificate (current within 2 years)

  • Willing to undergo Vulnerable Sector Police Check as a condition of hire

HOUSEKEEPERS

 •    Follow routine protocols to clean and maintain any required areas in the facility.
   •    Operating and caring for housekeeping machinery, including keeping work area clean/tidy, reporting repairs, requisitioning, and stocking the work area as needed.
   •    Safe handling of chemicals according to WHMIS guidelines, and ensuring all chemicals are kept in a locked area between uses.
   •    Protect confidential data or information regarding residents.
   •    Adhere to occupational health and safety standards, infection control guidelines, any legislation and sanitation codes, as applicable.

Qualifications:
   •    Minimum Grade 12 Educations/GED or equivalent
   •    Experience and knowledge in proper institutional housekeeping procedures an asset
   •    Must be available to work weekends, statutory holidays, and varied shifts
   •    Experience working in Long-Term Care field an asset
   •    Ability to work both independently and in a team environment
